What conditions do you treat?
I treat pain from degenerative joint disease, pain from osteoarthritis and rheumatoid arthritis, headaches, temporomandibular joint dysfunction, neck pain, rib pain/dysfunction, Low back pain/sciatica, joint pain/conditions of the shoulder, elbow, wrist/hand, hip, knee, ankle/foot, shoulder impingement, frozen shoulder, tennis/golfer’s elbow, sports hernia, hip impingement, patellofemoral pain syndrome, IT band syndrome, plantar fasciitis, rehabilitation after total joint replacements, rehabilitation after ligamentous surgery including ACL, rehabilitation after surgery including but not limited to meniscus repair/removal, rotator cuff repair, subacromial decompression, multimodal chronic pain management
Do I have to see my doctor first?
No. In Illinois I can evaluate and treat you for up to 15 business days without needing a doctor’s order for most insurances.
What insurance plans do you accept?
Blue Cross Blue Shield, Healthlink, Health Alliance, Medicare, Medishare, Tricare, VA, Cigna, Aetna, UMR, United Healthcare.
But how can you treat me if I haven’t had an X-ray or MRI to know what’s going on?!
Unless there has been a specific injury, imaging is often not necessary to begin effective treatment. With a thorough medical screening and examination, I can help determine if imaging might be helpful. Depending on the injury imaging is also not always necessary right away.
How much does physical therapy cost?
Some insurance plans have copays that are due at the time of the visit. These are generally $40 or less. This may be all you owe per physical therapy visit. Other plans don’t have copays but you will generally pay 10-30% coinsurance, which is a percentage of the negotiated rate between insurance companies and my contracts, due on a monthly basis.
I also offer out of pocket rates at $125 for new patients/conditions and $100 for follow up treatments. New patients are assessed and treated for 1 hour and follow up appointments are scheduled for 45 minutes. If you have been previously evaluated for a condition in the past year I offer 15 minute out of pocket dry needling sessions for $35.
What should I wear to physical therapy?
Wear something comfortable and easy to move in. Plan to have the body part/region exposed for which you are coming in for treatment.
